At your first appointment
During your first oral surgery appointment with Dr. Feldman, you will have an examination and consultation. We invite you to ask any questions you may have about your condition or procedure. We will collect your medical history, evaluate your overall health, and discuss your treatment options.

Pre-Registration
Prior to your visit, you can print and complete the following forms. Simply bring them to your appointment. Or arrive at least 15 minutes early to complete them in office.
Please bring with you
- Your surgical referral slip from your dentist (if applicable)
- X-Rays from your dental office (or have them forwarded to us prior to your appointment)
- A list of medications you're currently taking
- Insurance cards
- Health history and current health information, including medical conditions such as heart disease, diabetes, high blood pressure, etc.
All patients under the age of 18 will need to be accompanied by a parent or legal guardian.
Payment Options
We accept CareCredit, Visa, Mastercard, Discover and American Express.
Cancellations
We require at least 24 hours notice to avoid cancellation fees.
What if I my dentist just took x-rays?
If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.
Do you accept payment through flexible spending accounts (FSAs) or health savings accounts (HSAs)?
Yes, we accept FSA and HSA cards for payments. Please check with your benefits provider to ensure the expenses are eligible under your plan.
What happens if I’m running late to my appointment?
If you’re running late, please call our office as soon as possible. We’ll do our best to accommodate you, but depending on our schedule, we may need to reschedule your appointment to avoid delays for other patients.
